Space maintainers are an extremely effective orthodontic device that keep an emerging adult tooth from being overcrowded by surrounding teeth. They are custom-made by a pediatric dentist to fit between teeth in a child’s mouth. 

This appliance can either be metal or acrylic, in addition to being removable or cemented permanently. Usually the appliance can be installed in just one or two visits, and is a very simple procedure.

Are space maintainers really necessary?

Yes, especially if a baby tooth falls out prematurely! 

It’s important for emerging adult teeth to have the space needed to erupt where they’re supposed to. If they don’t, they can get blocked by surrounding teeth (impacted), potentially requiring braces or even an extraction down the road.

Many times the transition from baby tooth to adult tooth is smooth, not requiring a space maintainer. But sometimes a baby tooth can fall out prematurely from tooth decay or injury.

Since baby teeth serve as a guide for where adult teeth need to grow, a baby tooth that falls out prematurely can cause misaligned adult teeth and lengthy procedures in the future. Space maintainers are the perfect solution for when baby teeth fall out prematurely.

When the space maintainer needs to be removed depends on the type being used. Be sure to consult with your dentist for recommendations specific to your situation.

Sometimes a space maintainer isn’t necessary, but a pediatric dentist can help make that determination. It is recommended by the American Academy of Pediatric Dentistry that parents bring in their child every 6 months for regular checkups. Then we can see if your child’s adult teeth are erupting correctly.

Are space maintainers painful?

No they are not painful! Space maintainers (spacers) do not move teeth around like braces do, so there is no pain.

When that guide is removed too early, neighboring teeth tend to tip or drift into the gap, which can prevent the permanent tooth from erupting in its correct position. Not always. In some cases, the permanent tooth is close enough to erupting that a space maintainer is not needed. The determination depends on your child’s age, which tooth was lost, and how much time is expected before the permanent tooth comes in. Dr. Jobe will evaluate your child’s X-rays and dental development to make the appropriate recommendation. The American Academy of Pediatric Dentistry recommends checkups every six months, which is the best way to catch these situations early and respond quickly.

Space maintainers come in two main varieties: fixed and removable. Fixed space maintainers are cemented onto neighboring teeth and stay in place until Dr. Jobe determines they are no longer needed. Removable space maintainers look similar to a retainer and can be taken out by the child for cleaning. The best type for your child depends on which tooth was lost, the child’s age, and their level of cooperation. Dr. Jobe will discuss the options with you and recommend the most appropriate choice for your child’s specific situation.

No. Space maintainers do not move teeth the way braces do, so there is no pressure-related discomfort associated with wearing them. The placement process is straightforward and comfortable for most children. Some kids may notice the appliance in their mouth for the first few days as they get used to it, but this typically passes quickly without any pain. If your child reports ongoing discomfort, our St. Louis team is happy to take a look and make any necessary adjustments.

The process is simple and typically completed in one to two appointments. An impression or scan of your child’s teeth is taken so the space maintainer can be custom-made to fit properly. At the placement appointment, the appliance is fitted and either cemented into place or handed to your child with instructions for removable versions. The whole process is non-invasive and requires no anesthesia in most cases.

The length of time depends on how long it takes for the permanent tooth to erupt into the maintained space.
